Prior to your first visit, we will need basic demographic and background information and review your applicable insurance benefits and our fees.

WHAT TO EXPECT AT YOUR FIRST APPOINTMENT

We ask that you arrive on time for your appointment to complete our new patient forms.

Dr. Dadgar will review your medical history in detail, discuss your symptoms and condition(s) and perform a complete physical and pelvic examination. Options of management and a recommended treatment plan will be suggested. If additional procedures or tests are recommended, our Surgery Coordinator will assist you in scheduling and the necessary preparations.

WHAT TO BRING

Please print & complete the NEW PATIENT PACKAGE

We may ask you to bring medical records relevant to your gynecological care. These documents may include:

• Copies of any tests, x-rays or medical histories
• A list of all current medications, including over-the-counter (OTC) and herbal supplements
• Any specific directions related to diet or medication restrictions from other doctors
